Monthly Med Spa Membership Agreement

 

Membership Term

All memberships require a six (6) month commitment. Membership agreements must be renewed and re-signed at the end of each six-month term to continue benefits. Membership benefits are available only while the account is active and in good standing.

 

Payment & Automatic Billing

 It is the Member’s responsibility to maintain updated payment information. Membership fees are automatically charged on the 1st day of each month for the 6-month term and each subsequent term to the card on file. By signing this Agreement, you as the member authorize Amaka Aesthetics to maintain a recurring automatic subscription charge of $199/month. Declined or failed payments must be resolved within five (5) business days to avoid suspension of benefits.

 

Cancellation Policy

A 30-day written notice is required to cancel a membership. If cancellation notice is submitted less than 30 days before the next billing date, the Member will still be charged for the following month. If a Member cancels their membership, they will not be eligible to re-enroll for a period of six (6) months from the cancellation date.

 

Pausing Membership

Memberships may only be paused for pregnancy, medical illness, or military deployment. Medical documentation is required for all pause requests. Approval is at the discretion of Amaka Aesthetics management.

 

Transparency & Results Statement

Amaka Aesthetics is committed to providing safe, ethical, and medically appropriate aesthetic treatments. Results vary from patient to patient and depend on numerous factors including consistency, treatment plan adherence, lifestyle, and individual biology. Many aesthetic treatments require multiple sessions and time to achieve optimal results. Membership participation does not guarantee specific outcomes. Members acknowledge that visible and lasting improvements often require patience and ongoing treatment.

 

General Terms

Memberships are non-transferable. Monthly benefits (facials) do not rollover or accumulate. Amaka Aesthetics reserves the right to terminate membership for violation of policies, inappropriate behavior, or non-payment.

Please note, no services other than the ones listed below are included in the Amaka Aesthetics Monthly Membership subscription:

Dermaplaning, Microneedling, Medical Grade Chemical Peels, Facials, Filler, Neuromodulators, PRP, Laser Hair Removal and Aerolase treatments.
